Your eligibility for federal assistance is based in part on your cost of education, expected family contribution, and other sources of financial assistance you will receive.  Any change to these factors after your original award has been made may cause an adjustment to your eligibility and/or require repayment of some or all federal funds you received for which you were not eligible.

You will be notified in writing if these awards cause an adjustment to your federal aid eligibility.  If you learn of other resources, or if the award(s) you indicate change after you submit this form, you should immediately notify the Office of Scholarships and Financial Aid in writing.

Please indicate the name, agency, and amounts of assistance you will be receiving in addition to aid sources already on file with the OSU Office of Scholarships and Financial Aid.  We request your Campus-Wide ID (CWID) on this form to protect your privacy; if you do not know your CWID, you can logon to the secure OSU SIS-Web Student Self-Service site using your Social Security Number and PIN (default PIN is your birthdate).

I understand that these awards may cause an adjustment to any previously-awarded financial aid, and that I will be notified of any changes by email, and will need to review my aid information on SIS-Web for award amounts.  I further understand that this email will be sent to my 'okstate.edu' email address, and it is my responsibility to check my 'okstate.edu' email or forward email sent to that account to an alternate email address (if you have not yet been admitted to OSU, email will be sent to the address you listed on your application for admission/scholarship or Free Application for Federal Student Aid until you have been admitted and are assigned an 'okstate.edu' email address).
